Arapahoe Warriors Youth Hockey Association

Attached below is a confidential application for the 2011-2012 AYHL Booster Club Scholarship Fund.

Any awarded scholarship funding will be paid directly to the South Suburban Family Sports Ice Office to offset a portion of the 2011-2012 AYHL travel team dues owed by the individual player.

The maximum amount that will be rewarded per family is $300.00 in order to help out the maximum number of Arapahoe families.

The scholarships will be available to any travel team member in good standing with the Arapahoe Youth Hockey League. The member applicant must not be in arrears with AYHL for any previous season.

The player’s parent or guardian must submit the following documentation in order for a player to be considered for a scholarship:

• Completed Scholarship Application Form
• Copy of most recent paycheck stub
• Copy of most recent federal income tax return
• A written statement to the Scholarship Committee requesting financial assistance that includes the reason for the scholarship request and any other facts or circumstances to support the request.

The deadline for receipt of scholarship applications is October 15 of each year. All application forms and information submitted will be kept confidential. The Scholarship Committee will keep all documentation submitted, so please provide copies and keep all originals.

An AYHL Booster Club Scholarship Committee representative may contact scholarship applicants during the application process for additional information. No scholarship requests will be accepted after the due date of October 15th.

Scholarships will be awarded and members notified by October 20th.
